upgrading from OS 9 to OS 10.4.1

Help!!!!.... I just got back into supporting MAC Equipment... I just want to upgrade OS 9 to OS 10.4.1...Having "insert disk 2" messages during installation, once installed, it's bombs out on me.. Could it be Firmware upgrade needed????

It's hard to troubleshooting advice when there is no other info supplied about the Mac you have.

What year, CPU speed, hard drive size and amount of RAM installed?
